Tiff Does A Review: The Icon Clinic - My Alarplasty Journey

Hello everyone!
Be aware that this will be a very long wordy post with lots of graphic pictures.

I know this may be shocking to a lot of you but don't panic - I'm absolutely fine!
Here, I am sharing with all of you my personal experience of having Alarplasty done on myself. In simple words, it is a 'nostril reduction' cosmetic surgery.

The surgeon will slice a wedge out to slightly pinch the wide nostrils in to appear smaller. Fortunately for us, we have advanced technology and modern, updated ways of plastic surgery now, the old school methods are long gone, so such cases of having the Michael Jackson's nose shouldn't be a problem. That method was done way back in the early 80's.

TAKE NOTE: Make sure to ALWAYS do your research and go into forums for more information about all the surgeons you've talked to. It's always safe to get assurance and verification before confirming with your chosen surgeon.

As for me, I didn't want it to be altered in an obvious form or have unrealistic expectations regarding my nostril enhancement, I just wanted it to be subtle, to fit my face harmoniously and have it look as natural and effortless as possible. I still wanted to look South East Asian and not make my physical looks set apart from the rest of my family members, meaning, I still wanted to keep it 'ethnic'. Haha! XP

It took me years to do a lot of researching since I was in my teens. I have always been a skeptic - afraid of morphing myself to look like someone else and immediately brushed off the idea of having it done on myself, but I always wondered how it would look like if I got it done.

Well..

IT'S THAT SAME OLD STORY.

I guess getting picked on played a big role on me making this big decision. One of the reasons for my decision is to bust a cap up their asses to shut their pieholes up, at least I'm doing my best to be a better version of me. (I told my brother, Shaz, that I'm trying to be like Ironman, since there are many versions of Marks like Mark I, Mark II, Mark III and etc. He laughed along with me when I gave that reason as to why I feel like improving myself. LOL!)

Everyone would say "It's mind over matter, Tiff" but it felt like all I wanted to do was to get it over and done with. So, when the time felt fitting for me to do it, I just thought "Why not?!".

Another reason that got me even more convinced was when I read on some articles online about how our noses won't ever stop growing and I already have a nose that never stops growing wide. Lol!

It was always a sign when I came across online reviews and articles on it. Almost like giving me the clear greenlight to tell me it was okay to get it done.

So, it definitely felt like everything fell into place, especially when I came across a surgeon who has a lot of great experiences and his work portfolio is remarkable! I was highly impressed by his caring nature and the way he gave his assessment about my nose was EXACTLY how I felt about it too. Out of all the surgeons I've talked to for a few months, he stood out as the most knowledgeable and showed me that he is committed to balance out the harmony of my nose exactly the way I wanted it to be! We were seeing things eye to eye so that truly settled it. He was available for me through Email AND Whatsapp, so he's always within reach anytime I needed reassurance!

My surgeon is Dr Eric Yapjuangco from The Icon Clinic.


I chose Manila as the place to go for my alarplasty surgery because of how common it is to get it done there. I have personally heard from acquaintances who have undergone plastic surgery and read forums on how well experienced the surgeons there are. I could've gone to Thailand, but I just wanted a surgeon who is very accommodating and has done alarplasty to countless of patients.

And you don't ever see a Filipina celebrity who doesn't look good after having something done to herself. They're always on point about their plastic surgery!

Here comes the pictures!

WARNING: Some of these pictures ARE NOT for the weak! Stop HERE if you do not wish to see blood, stitches, swells, scabs and etc.

This is the lobby upon entering the building.
The clinic is on the same floor as the lobby, so it wasn't hard to find it.
I had myself registered and checked in.
The Med Tech, Joane Aimee, took some blood out to check that everything is okay with me. For the first time ever, I found out that I'm a blood type 'A'. 
I had an hour of free time before my surgery, so I had some latte to calm my nerves down.
Then, checked in to one of the resting wards for pre/post surgery patients.
I became VERY VERY nervous. I started getting jittery and I could feel a sweat building up. Haha. Good thing for free WiFi in this resting ward, my loved ones in Whatsapp were supportive and kept praying for me! <3 Thank you, everyoneeee. I love you alllll!
Doc and his wonderful nurses were EXTREMELY accommodating and helped to calm my nerves down. Doc asked me if I have any music I wished to listen to while undergoing surgery and I said "Anything R&B, from Boys II Men to Jhene Aiko, old school or new is fine with me".

HOW COOL IS HEEE? Lol XD

I enjoyed the preparation of the surgery. Nurse Cyril helped to clean my nostrils out and trimmed the hairs very short. She, then, brushed a type of liquid mask on my face, neck and chest to make the surface of the skin firm. It was very relaxing!

Doc played songs from Boys II Men, Ne-YoMario to John Legend as he was doing measurements and markings on my face. I enjoyed all of the process so far.

The only part that was very painful was when he injected the local anaesthesia in, a few minutes later, I was SO numb that if you threw a brick at my nose, I could feel absolutely nothing (>_<)

I was practically humming and singing along to every song as he was slicing my nostrils with a scalpel. When the left nostril was sliced open, I swear I could feel cold air rushing in and could breathe in so much oxygen! LMAO! XD

There was a point of time I even asked, "Are you sewing me up now, Doc?" and he would say "Yes, I am!". HA!

So, it was definitely a PAINLESS procedure.

After 1 hour and 15 minutes, my surgery was done. My heart jumped for joy seeing how it looked like without any swelling yet! I love how it looks, not too different and very natural looking!

But damnnnnnn, I got VERYY swollen after tho! I still am very swollen now as I'm typing this.

I still had markings on me that took about 2 days to completely fade out.

During the first day, the swelling continued to build up and the anaesthesia was subsiding later in the day. Only this time, I felt the pain coming in, thankfully, Doc prescribed me with Ibuprofen as pain killers and pills to help the swellings to subside.
Doc gave me treatment gels needed for anti-bacterial and anti-scarring; the reason why the pictures of my nose are mostly shiny (products of treatment gels will be pictured at the end of this post)
I had a follow up 3 days after for Doc to assess my stitches. It was obviously too soon to take them out.
The stitches were not comfortable at all.
I took my stitches out 5 days after and I felt so relieved!




After  a week, I still couldn't smile yet, so I have with me a serious, stiff face everywhere I go, even right now. LOL.

I was walking around to work with a face mask or a bandage on but after a week, I just couldn't handle it anymore and stopped wearing it, it was before reaching my 2 weeks post op, because the weather here gets so humid and nasty. It was getting annoying!
And slowly, the scars left were gradually healing. It takes time for it to disappear, even up to 3-6 months.

My nose bridge area is still swollen and very firm to the touch. My nostrils and the tip of my nose are still somewhat numb, like if I put the tips of my fingers on it, I can't feel my nose at 100%.
My latest pic below. I still can't smile at 100% and feel stiff, but was able to wear makeup slightly closer to the nose now. Yay!
These are the treatment gels:

1) Terramycin (Anti-bacterial gel)
2) Dermatix (Anti-scarring gel)

It will be swollen for around 3 months and only after that period ends I can finally see the final outcome of my alarplasty surgery

This post is getting long, so I'll have to stop here for now.

    1. It is a minor surgery, so yes, you will still be able to walk and move around. It all depends on how you can handle it after the surgery. Take note that 2-3 hours after the surgery, your local anesthesia will subside and you will have to take your medications and painkillers instructed by doc, of course. Take care!

  3. Is it possible to feel any dizziness or vomitting after the surgery? Because i hope that after two days, i will be able to go out and process some documents in public places like city hall, i'm also worried that it might get infected if it's exposed to some dirt although i'm wearing a mask. Does the stitch really look bad that you'll feel awkward when somebody sees it? Thanks. Sorry if i have a lot of questions.

    1. HI Minchae! I'm glad to be of help!

      Personally, I didn't feel dizzy or had any uncomfortable feeling after my surgery, my nose just felt stiff. Doc's assistant helped me hail a cab straight to the hotel. Yes, you may go out and about after 2 days, either expose it or just wear a mask because after 2 days, it'll still be bloody and scabby. Don't worry about the dirt, Doc will give you samples of Dermatix and Terramycin, make sure to saturate the stitched areas with these gels. You'll still get looks with a mask on or without it. Just do you, girl! People in Philippines are nice and plastic surgery is common there, where the city is has a few PS clinics so it shouldn't be a problem :D

      The hotel I stayed was soooo convenient, it was easy for me to get errands done and to shop for food and stuff at the pharmacy.

      Thank you for dropping by my blog. I don't quite understand what you're saying but I can kind of figure out a part of the context is a question about the cost of surgery. (Sidenote: I'm not from the Philippines nor am I a Filipina)

      It ranges between PHP20,000 and above, it really depends on your nose. My best advice is for you to ask the quote personally from Doc himself cos all noses are different. However, mine was exactly PHP20,0000.

    1. Immediately. Best applied on a fresh wound so the scar would not be so apparent. I doubt that the nose could ever go back to its former shape and size because Doc would really slice a wedge of the alar's skin out. Hope this helps! xo

    1. I applied the terramycin first (it's anti bacterial). Wait 1-2mins. Then, apply a layer of dermatix. It's all about layering! :)

  9. You're really a big help thanks a lot! I also had my alarplasty done on jan.5 and it's my 2nd week now :) i really feel uncomfortable because my nosetrils are still numb and swollen so i can't appreciate yet the result of my surgery. I feel pain when i sneeze or yawn though i've been trying to avoid sneezing and yawning :(

  11. Hi Tiff, thank you for sharing your story. It's very inspiring to me because I wanted to get my nostrils done too. Can you explain why there are the horizontal cuts on both side of the columella? I always thought that the incisions are made only the outer part of the nostril where the crease is but you had it cut from the inside too. So each nostrils have 2 sutures? What is the purpose of the inside suture? Please give me clarification. Thanks in advance!!

    1. Hi Myb8b8! Thank you for taking the time to read my blog! Doc did that because I had nostrils that overhang and flare downwards, which hid my columella. In order for the side profiles of my nose to look more balanced, he did that so that my nose won't appear as round and bulbous looking like before. I LOVE IT tho! Hehe! When are you planning to get yours done?

How many follow up visits did you had with him? Was the hotel and stuff expensive? Can you please provide a close up front & side view of your nose (if possible)?     1. Hi Ju'en! I don't have the specific details for how much in mm did Dr Eric took out. I was shown the bits that he took out, it was quite a big difference, around 1/4 to almost 2/3 of skin was took out from my alars. It was okay to wear makeup immediately, but not on the fresh incisions which were still healing, you may start wearing full makeup after 2 weeks r so. I washed my face with a wet towel gently on the sides of my face and use a q-tip around my nose :) Thanks for dropping by my blog and for reading this post! xo

      I would assume by 3 weeks you would be less stiff. As for me, I could still giggle and laugh gently. However, I was not able to feel my nose at 100% yet. It really depends on each individual's experience as our bodies will react somewhat differently.

      It really depends on your concerns and how the doc analyze it.

      Usually cases like wide flaring noses, overhanging alars or low bridged noses are eligible for the nose surgery procedures.

    1. Hello hello! Thank you for visiting my blog. It depends on the individual. Drink water, avoid alcohol/sugary/salty foods and rest rest rest. It takes up a whole year for the nose to heal completely. The areas that have been cut will be stitched, then taken out 5-7 days later, then it will bleed a bit as it heal and scabbed over after 2 weeks. Probably 2-3 months for it to look normal again. Depends on how you take care of yourself.

    1) Doc did an incision on the nostril's line in between the nostril and the face.
    2)My breathing has improved A WHOLE lot more if anything :D yay!
    3) Being nervous is normal, I was a sweaty nerveball on my surgery day but everything went by so fast and the procedure went smoothly.

    Noticed you only had alarplasty and not tgtr with tipplasty or bridge augementation?
    Any reason?

    Dr Eric suggested me to have a tipplasty as well as bridge implant but i dont want to put foregin object into my nose or drastic changes so im sticking to just alarplasty.

    Does alarplasty alone make your tip look bigger? or your nose odd?
    Sorry for the many questions. Just want to equip myself with enough info before proceeding with the surgery.

      I would strongly advise you to consider Doc's suggestions.

      I only had alarplasty done with Doc Eric from The Icon Clinic but years ago, I also had my bridge raised with a silicone L implant (sized Small) when I was 20 at a local clinic in Malaysia.

      If you only had your alars reduced, the harmony of your whole entire face may look 'off', but do check with the experts first okay? xo

      Yes, if you're just getting nose surgery, a month is more than a good amount of time to fly back to Qatar. Recovery doesn't take long, depends on how many surgery procedures you're planning to get.

      My pain tolerance is between medium - high.

      (Pain level description: 1 being painless and 10 being very painful)

      On Day 1: The part when Doc injected the local anesthesia around my nose was pretty painful (I teared) Pain level: 8

      After the LA kicked in, the surgery itself didn't hurt at all and I can feel the motion of Doc slicing through my alars. Pain level: 0

      After a few hours, the LA wore out. I made sure to be in bed, prepare ice in a towel and have painkillers with me to ease the pain. Pain level: 7

      Day 2: Slept peacefully maybe cos I was tired. Woke up before noon to have breakfast, I can still feel the throbs of pain around my nose but I was able to endure it as it wasn't as painful as the day before. Pain level: 5

      Day 2: Once the areas where incisions were done scabbed over, the pain has lessened: Pain level: 4

      Day 3 onwards: Some parts of my nose felt numb and the tender areas are scabbed, so it felt okay. I just couldn't smile or laugh that much. Pain level 2-3

      After a month: I could no longer feel the soreness of the swollen parts.

      After 3-4 months: I can feel the sensation starting to come back to being normal.

      Around 6 months: I could finally laugh out loud and smile wide without worrying.

      A year on: No pain, no worries and no regrets.

    1. The thing about surgery is really about preparing yourself mentally, physically and emotionally. It took me years to finally decide on this after researching what I need to expect online. It's really mind over matter, just have to brave yourself. Anything with scalpels and blood will always sound like it's a painful procedure, just come prepared and ready.

      The removal of my stitches wasn't painful, just uncomfortable. Cos it felt like some foreign thing was hogging on to my nose. Always clean up any dried sebum/blood with a q-tip each time you see it appearing around your nose, too many of this will make it difficult and messy for the stitches to be removed.

      It's normal to be curious and worried about PS but you're doing the right thing by educating yourself. Don't go for surgery until you're 100% ready. Take care. xo

      I didn't ask how much of my excised flesh have been taken out, however, I did see the bits and pieces of my flesh on a metal plate after the surgery was over, it wasn't that much tbh. Just like teeny pieces.

      I would lie if I said I wasnt! However, I came fully prepared mentally and emotionally ready! Also, doc was kind and thoughtful to turn on R&B songs for me to feel comfortable. Building complete trust with your doctor is important too. Hope this helps! xo

Emulating this, your surgeon will no doubt request you to wear a nose brace or tape for a specific period of time. Once you're feeling more like yourself and are on betterment track, it might be enticing to scrap the bandage rashly.

He would ask those patients to come back after a year for revision. Worst is that you cannot talk to Eric Yapjuangco after the surgery since his assistants who are wearing shirts with "ICON DOLLS" logo are the ones who will attend in removing sutures, I'm not even sure if they are nurses, some look like their maids. Eric Yapjuangco should be avoided if you want to save your life. The pictures that he posts are even edited. Eric Yapjuangco frequently creates piglike noses no wonder he looks like a PIG, or a fat version of PSY. Stay away from him please, and dont delete this admin if you want to be fair..

  71. sorry but for me he's a botched king he made my nose worst. He's the reason why I have been so depressed right after the surgery. He made my nose too big by putting in a huge implant without carving it properly it's now very visible and looks unnatural, not to mention the very obvious scars on alars and nostrils. I wonder how did this doctor become a plastic surgeon? He's so amateur. I have been searching for the expert doctor who would revise the botched nose job Dr Yapjuangco has done. I have not been eating well I have been depressed I look so different now very ugly.
